Dear Represenative ________ / Dear Senator ________ ,

I am very concerned about health care reform. We need reform that will ensure high quality, universal health care that really covers everyone. Reforms must reduce cost in the long term. Good coverage and choice should be provided, no matter what a person’s income level.

Add one or two details about what kind of reforms you want. For example (edit as necessary):

* It is important to me that a strong public plan is part of health care reform. Without a strong public plan, reform will not work.
* I believe that a “single payer” national health program is the only way to provide truly universal care while also maintaining quality and reducing costs. I hope that the upcoming reforms will be a step in that direction.

This is a very urgent issue. [Tell a brief story about your own problem with the health care system, or the problem of a friend or relative.]

Sincerely,
